If your complaint involves a doctor, dentist, or pharmacist

If the reason for your complaint involves a doctor, dentist or pharmacist, the service quality and complaints commissioner will make sure that it is sent to the establishment's Medical Examiner of the CIUSSS de la Capitale-Nationale, as required by law.

Although the Commissioner does not investigate complaints about medical acts, she collaborates closely with the Medical Examiner and is available to keep you up to date on the progress of the investigation.

How to file your complaint with the Commissioner

The Service Quality and Complaints Commissioner Ms Dominique Charland. There are a few ways you can file a complaint.

  • Over the phone by calling 418 691-0762.
  • In person, but please call Ms Charland first to make an appointment at the above number.
  • In writing: if you wish, you may use our complaints form (PDF of two pages in French and English) and then send it in one of the following ways:
    • by fax: 418 643-1611
    • by email: commissaire.plainte.ciussscn@ssss.gouv.qc.ca OR
    • by regular mail to the following address:

Response time when treating your complaint

After their inquiry, the Medical Examiner must give you their conclusions within a delay of 45 days after having received your complaint as required by law.

If you have not received a reply within the required delay, OR you are not satisfied with the results, you may request the opinion of the Revision Committee of CIUSSS de la Capitale-Nationale in writing:
