Medical Records Service

At Jeffery Hale, we keep track of all of the tests and treatments that you get under our roof.  If a professional  from another health establishment, such as a doctor, needs certain information about you from your Jeffery Hale file, we can send it to them with your permission. 

Likewise, if a medical professional working at Jeffery Hale needs information about your medical history from another health establishment, we can obtain that information at your request.

Access to medical records

Sections 17 to 28 of the Act respecting health services and social services (R.S.Q., chapter S-4.2) guide establishments on issues of confidentiality and access to medical records. Other laws, such as the Act respecting access to documents held by public bodies and the protection of personal information (R.S.Q., chapter A-2.1) and the Youth Protection Act (R.S.Q., chapter P-34.1), also influence confidentiality and access to medical records. Given the complex nature of these laws, Jeffery Hale handles each request individually. Our medical records administrator is responsible for applying the rules governing access to your record, in keeping with these laws.

Also, please note the following:

  • We do not give users any information over the phone, except the name of the attending physician.
  • We do not send any information to users or their representatives by fax. 

How do I access the information in my medical record?

You cannot leave the establishment with your medical record. To receive a copy of information contained in your medical record, send a written request to Jeffery Hale’s Medical Records department.

To do so, you can use the following form: Authorization to Release Information Contained in the Medical Record (PDF).

Send your completed form to the Medical Records department by fax (418 684-2281) or by mail (Jeffery Hale – Medical Records, 1250 chemin Sainte-Foy, Quebec City, QC G1S 2M6).

or

Write a letter to the Medical Records department providing the following information:

  • Family name and first name
  • Date of birth
  • Health card (RAMQ) number and/or Jeffery Hale Hospital Card number
  • Mother's family name and first name
  • Specify the type of information required and the date
  • Signature
  • Specify the name and address of where you want the information to be sent (ex. to yourself or to your doctor)

We require a two- to three-week delay to respond to written requests. 

You can also drop off your written request in person at Jeffery Hale, Monday to Friday from 8 a.m. to 12 p.m. and from 1 p.m. to 3:30 p.m. (open level 1 floor map).

Do I need to pay to access my medical record?

There are no fees for accessing your medical record. You simply need to make an appointment with the Medical Records department.

If you would like a copy of documents contained in your medical record, you may be required to pay photocopying and delivery fees. We will let you know in advance if these fees apply.
